December CPI registration assistance

Complete one submission per attendee using that person’s own email address. Include your membership and scholarship status so Karla can review eligibility and arrange the appropriate invoice. This form does not reserve a seat, collect payment or confirm enrollment. Do not also pay through CMHA unless WHA directs you to do so.

WHA uses the information you provide to respond to your inquiry and coordinate your request. Do not include payment details or sensitive personal documents. Sending this form does not sign you up for marketing messages.
